Abstract

Introduction: In the last 75 years, we have observed a 15 fold increase in the number of individuals over the age of 60 in Brazil. This trend of population aging is likely to increase in the coming years. It is known that elderly people have peculiarities in relation to pharmacotherapy with a higher risk of drug interactions. The concept of potentially inappropriate medications (PIM) for the elderly emerged with the aim of preventing drug iatrogenesis in this group. Objective: To analyze the prevalence of PIM use in elderly people treated at a geriatric outpatient clinic in Feira de Santana, Bahia, according to the Beers criteria and the Brazilian Consensus on Potentially Inappropriate Medications for the Elderly. Methods: This is an observational, descriptive and cross sectional study. We carried out, through a structured questionnaire, a detailed medication survey, in addition to recording sociodemographic data, lifestyle habits, history of falls and hospitalizations. Results: Of the 97 patients included in the study, 54% used some PIM. Antipsychotics and benzodiazepines were the most common PIMs. The prevalence of polypharmacy was 78% in our sample, a high percentage compared to other studies. Patients using 4 or
more medications used 3.5 times more PIM than those using 3 or less (RP 3.529) (CI: 1.281 to 12.64) (p = 0.0108). There was no significant difference in the prevalence of PIM when using the Beers criteria or the Brazilian Consensus on Potentially Inappropriate Medications for the Elderly. Conclusion: Our findings suggest a high prevalence of MPI in elderly patients managed on an outpatient basis. A positive association was observed between the number of medications in use and the prevalence of PIM. There is a need for greater awareness among the medical community about the risks of introducing this type of medication in elderly patients due to their greater susceptibility to its adverse effects.
